Client Journey and Information
We understand the significant impact pain and injury can have on your life and aim to make your journey with us as smooth as possible while providing an optimal outcome.
Here is everything you need to know before your upcoming appointment with us.
Follow the steps below to ensure you get the most out of your appointment.
Make an Appointment
Book online to make an appointment. You can select your preferred therapist and location while booking.
Alternatively, call our friendly admin staff on 0422 483 865 for bookings and enquiries.
New Client Form
You will receive an email with our online form which we ask that you fill out before your appointment. This helps us prepare for your appointment and ensures that we can spend as much time as possible getting to know you and your injury.
Please let us know if you don’t receive your new client form. Sometimes they do end up in junk mail or under the “promotions” section if you are a Gmail user.
Prepare For Your Appointment
On the day of your appointment, we recommend wearing training clothes, or something you can move in easily which allows us to see the affected area.
We also ask that you bring the following to your appointment:
Your private health card, we accept digital and physical.
Any scans, doctors letters or other relevant documentation.
Most of our Physiotherapy treatment plans and programs are delivered via Physiapp, if you have a chance to download this before your appointment this will help streamline the process.
How to Find Us
We are a bit different to other physio clinics as we operate from within gyms. Please don’t feel intimidated - everybody is incredibly friendly and will direct you to the right place if you’re looking lost! Give us a call if you are unsure.
As we are based out of multiple locations, please ensure you attend the correct location as stated on your confirmation text and email.
Select the appropriate option below to find out more about your chosen location.

Your Appointment
You can expect us to conduct a full subjective assessment where we will ask you questions about yourself, your health and your goals. This will be followed by a physical examination where we will conduct tests to look at your injury and how you move.
Our treatment is guided by both evidence-based practice and your individual preferences and goals. Treatment can come in the form of exercise therapy, massage, dry needling and taping. We will also provide education and advice on how to manage your condition.
After Your Appointment
Our team and your Physio will work hard behind the scenes to ensure that you can make the most progress possible between your appointments.
You will receive an exercise program via Physiapp if applicable, and a personalised treatment plan to map out your individual patient journey. Make sure to check your emails so you can access your exercise program and treatment plan!
We will also communicate and liaise with your coach/GP/specialist as required.
You can also contact your Physio about your condition either via Physiapp or their email.
For any appointment or invoice related questions please contact our admin team at admin@formotionphysio.com.au or 0422 483 865.
