Formotion Physio has experience in helping you navigate through WorkCover / Workers Compensation claims at any of our clinics in Osborne Park, West Perth and Joondalup.

Key Points:

  • Physiotherapy may be provided to workers who have sustained a workplace injury, this is funded through the workplace’s insurance cover.

  • You’ll need a GP referral, claim acceptance letter and claim number.

  • Appointments for your specific work related injury are fully covered by the insurer.

  • You can book online by selecting a “Workers Compensation / Motor Vehicle Accident - New Assessment”

    or contact the clinic to make a booking.

How do I get started?

If you have sustained a workplace injury you will first need to talk to your employer and adhere to the Worker’s Compensation guidelines of your work place.

Once you have done this you will generally need a referral and a first certificate of capacity from your GP.

You will need the following details in order to start your physiotherapy treatment under Worker’s Compensation:

  • Claim acceptance letter from your relevant insuring company from work.

  • Claim details – including claim number, case manager name and contact details.

If you do not have this information you can still pay for your appointments privately. In some cases you may be able to claim these appointments back later - but make sure you check this first!

Who is responsible for bills?

Formotion will liaise with your case manager from the relevant insurance company to seek approval of physiotherapy treatment plans under the WorkCoverWA rates.

Once approval has been granted, all fees will be directed to the insuring company with no out of pocket expense for clients. However, any treatment outside of the proposed plan will be your responsibility.

Do I need to stay with the Physiotherapist I am referred to?

No! If you would like to change providers for whatever reason you should talk to you case manager to discuss this. If you have a provider you would like to you use you can discuss this with them prior and they may be able to help you with the changeover process as well as obtain any relevant handovers from the previous Physiotherapist.

Cancellation Policy

It is important to note that missed appointments and late cancellations fees will apply as per our cancellation policy. These are not covered by WorkCover or your insurance company and will need to be paid for privately if you miss an appointment or cancel late.
