What is Chronic Disease Management?
A Chronic Disease Management (CDM) Program is a means for General Practitioners (GPs) to assist in the management of the health and well-being of patients with chronic or terminal medical conditions.
You may be able to get a GP referral for up to 5 Physiotherapy / Allied Health sessions per calendar year if you have a chronic medical condition (a medical condition that has been or will be present for a minimum of 6 months).
This was previously known as EPC plans - Enhanced Primary Care Plan.
How do I get and use a CDM plan?
You will need to see your GP for a referral before seeing a Physiotherapist / other Allied Health professional. They will provide you with a care plan which you can then bring to your Physiotherapy session.
You will need to let us know prior to your appointment that you have a CDM plan so that we can use the correct fee codes and have all your details ready to go.
How much do I pay?
Medicare funds a portion of these sessions. The rebate is currently about $60.35 per session.
We charge our usual rates (click here to see our fees) and then automatically process your rebate which normally appears in your account after ~2 days. So you won’t need to go into medicare or sit on the phone for hours!

We cannot bulk bill this fee as the amount covered by medicare is unsustainable for us with the level of service and time we provide in our sessions.
